CYP3A4
سیتوکروم P450 3A4 (به اختصار CYP3A4) (عدد گروه آنزیم 1.14.13.97) یک آنزیم مهم در بدن است که عمدتاً در کبد و روده یافت میشود. این آنزیم مولکولهای آلیِ خارجیِ کوچک (بیگانهزیستها) مانند سموم یا داروها را اکسید میکند تا بتوان آنها را از بدن خارج کرد.
در حالی که بسیاری از داروها توسط CYP3A4 غیرفعال میشوند، داروهایی نیز وجود دارند که توسط این آنزیم فعال میشوند. برخی از مواد، مانند برخی داروها و فورانوکومارینهای موجود در آب گریپفروت، با عملکرد CYP3A4 تداخل میکنند؛ بنابراین این مواد باعث تقویت یا تضعیف عملکرد داروهایی میشوند که توسط CYP3A4 اصلاح شدهاند.
CYP3A4 از خانواده آنزیمهای اکسید کننده عضو سیتوکروم P450 است. چندین عضو دیگر از این خانواده نیز در متابولیسم دارو نقش دارند، اما CYP3A4 رایجترین و پرکاربردترین آنهاست. مانند همه اعضای این خانواده، این آنزیم یک هموپروتئین، یعنی پروتئینی حاوی یک گروه هم با یک اتم آهن است. در انسان، پروتئین CYP3A4 توسط ژن CYP3A4 کدگذاری میشود. این ژن بخشی از خوشه ای از ژنهای سیتوکروم P450 روی کروموزوم 7q22.1 است.

Drug Interactions: Nifedipine is mainly eliminated by metabolism and is a substrate of CYP3A. Inhibitors and inducers of CYP3A can impact the exposure to nifedipine and consequently its desirable and undesirable effects. In vitro and in vivo data indicate that nifedipine can inhibit the metabolism of drugs that are substrates of CYP3A, thereby increasing the exposure to other drugs. Nifedipine is a vasodilator, and coadministration of other drugs affecting blood pressure may result in pharmacodynamic interactions.
